CCCC has three membership categories:

  • Individual – $50 per year – Click the "Join Now" button at the top of the page.
  • Community Coalition – $100 per year.
    This category is for community-based coalitions working to improve end-of-life care at the local level.
  • Organization – Includes individual membership benefits for five staff.

Get Involved!

CCCC has workgroups that focus on specific topics related to end-of-life care and provide input on CCCC’s work. Participation in the workgroups is open to all interested members.

Current workgroups include:

  • Long-Term Care
  • Public Engagement
  • Developmental Disabilities
